Essure®-sterilisatieveertjes. Een literatuuronderzoek

Abstract

Essure® is een implantaat om vrouwen te steriliseren. Kleine metalen veertjes worden in de eileiders gezet, waardoor ze dichtgroeien. Sommige vrouwen met Essure® melden gezondheidsklachten. Bijvoorbeeld pijn, vermoeidheid, hevige bloedingen en geheugenverlies. Soms zijn de klachten zo ernstig dat de gynaecoloog het implantaat verwijdert. In Nederland hebben sinds 2002 naar schatting 30.000 vrouwen dit implantaat gekregen. Sinds juli 2017 wordt Essure® niet meer in Nederland verkocht. Het RIVM onderzocht of metalen die uit Essure® kunnen vrijkomen mogelijk schadelijk zijn voor de gezondheid. Gekeken is wat hierover in de wetenschappelijk literatuur bekend is en zijn experts en interne deskundigen bevraagd. Het is onwaarschijnlijk dat deze metalen schadelijk zijn voor de gezondheid, concludeert het RIVM. Veel implantaten, zoals heupimplantaten en stents, zijn net als Essure® gemaakt van metalen. Als metaal in het lichaam wordt gebracht, is het normaal dat daar metaaldeeltjes uitkomen. Doordat Essure® een klein implantaat is, gaat het om kleine hoeveelheden metalen. Verder blijkt dat er weinig wetenschappelijk onderzoek naar is gedaan. De literatuur die er is, wijst er niet op dat deze metalen schadelijke effecten voor de gezondheid kunnen hebben. Grotere studies met een goede studieopzet zijn nodig om een verband tussen metalen uit Essure® en deze gezondheidsklachten echt uit te sluiten. Het RIVM heeft ook gekeken naar de mogelijkheden om Essure® te verwijderen en of daarmee de klachten verminderen. De klachten blijken in dezelfde mate te verminderen bij de verschillende operatietechnieken om Essure® te verwijderen. Volgens experts is er daarom geen reden om de behandeling in Nederland aan te passen. In Nederland worden bij voorkeur beide eileiders met de Essure®-implantaten weggehaald. Dat is naar schatting bij ruim 4000 vrouwen in Nederland gedaan. Dit onderzoek is in opdracht van de Inspectie Gezondheidszorg en Jeugd (IGJ) uitgevoerd.

Essure® is an implant used for female sterilisation. Small metal coils are placed in each of the fallopian tubes to induce closure of the fallopian tubes. Some women with Essure® have reported health complaints such as pain, fatigue, heavy bleeding, and memory loss. Sometimes the complaints are serious in such a way that a gynaecologist removes the implants. Since 2002, an estimated 30,000 women have received this implant in the Netherlands. Since July 2017, Essure® has not been sold in the Netherlands. RIVM has investigated if metals released from Essure® could be harmful to women's health. It reviewed existing scientific literature and consulted experts and in-house specialists on the subject. It is unlikely that these metals are harmful to women's health, concludes RIVM. Many implants, like hip implants and stents, are made out of metals, like Essure®. The introduction of metals into the body typically results in the release of metal particles. As Essure® is a small implant, the amounts of metals released are minimal. Additionally, the investigation highlighted a lack of extensive scientific research in this area. The available literature does not indicate that these metals could be harmful to health. Larger studies with robust study designs are needed to definitively rule out a connection between metals released from Essure® and the reported health complaints. The RIVM investigated the possibilities of removing Essure® and whether this reduces the health complaints. The health complaints decrease to the same extent with the various surgical techniques to remove Essure®. According to experts, there is no reason to modify the current treatment approach. In the Netherlands, it is the preference to remove both fallopian tubes with the Essure® implants. In the Netherlands an estimated 4000 women have had Essure® removed. This investigation was commissioned by the Health and Youth Care Inspectorate.
